STUDENT DISCIPLINE MATRIX
FOR GRADES 3-5

Most students are very serious about wanting to take full advantage of the time they spend in school. They are conscientious about their academic endeavors and their school behavior. They have a good attitude toward school. Students who come to school to learn and participate in school activities seldom have problems with school rules. 

District 45’s Student Discipline Matrix
District 45’s Elementary School Student Discipline Matrix applies to all areas of the school, bus and while students are traveling to and from school. This discipline matrix is designed to provide guidelines for elementary school staff across the district and information to students and parents/guardians.

Consequences
Consequences represent the range of discipline options available to staff members. Staff members may consider the behavior, as well as whether or not it is repetitious in nature. Staff may choose to use a combination of consequences i.e. natural consequences plus phone call home. Involvement of support staff (administrators, Student Assistant Team members, social worker, psychologist) is also available when necessary, to develop a plan of improvement.


These guidelines are not all inclusive. A student who commits an act of misconduct that is not listed will still be subject to disciplinary action. These guidelines are meant to serve as guideline and are not to be considered as limiting discretion in instances with aggravating or extenuating circumstances. Disciplinary action may exceed or be less than the prescribed action at the discretion of the administration. Police contact may be made as deemed appropriate by administration. MATRIX OF BEHAVIORS AND CONSEQUENCES
FOR GRADES 3-5

Level One Behaviors and Consequence Matrix
(Examples of Nuisance Behaviors)
All handled by the classroom teacher Administrators are available for problem solving with teachers.
Minor misbehavior in hallways, stairs or bathrooms
Littering inside school or outside on school grounds
Use of inappropriate voice level
Loitering (not lining up when the bell rings, hanging out at bathroom, waiting in hallway/stair area instead of returning to classroom)
Wearing inappropriate clothing or attire (caps, bandanas, chains, shoes with roller wheels)
Cell phone not turned off in the building and/or secured in backpack
Possession of items that are disruptive to learning including Electronic Devices
Disrespectful use of classroom property
Out of seat without permission
Cheating / Forgery
Talking out of turn, Making Noises
Minor peer conflicts (typical age behaviors)
Lack of homework or class work completion
Minor playground incidents during TSPE
Using other’s materials without permission

(Behaviors disruptive to the entire learning community)
Verbal acts of misconduct, including but not limited to: Swearing, Name calling, Teasing, Put downs, Dishonesty D + E to L
Throwing or shooting objects, including but not limited to:  Snowballs, Ice, Rubber bands, Paper clips, Woodchips, Rocks, Food B to J
Physical acts of misconduct including but not limited to:  Hitting, Kicking, Shoving, Tripping, Spitting C to L
Defiance of authority towards any staff member B to L
Found in unauthorized location E to I
Bus Misconduct A to I + Q

Possession of a weapon (including toy weapons) G + L to P
Fighting & Aggressive Physical Behavior (where students exchange blows (hitting/kicking) in a state of anger Physical, verbal, emotional or sexual harassment to an adultwith an intent to injure) G + L to P
Threats, Harassment, Intimidation or Racial Comments to Student or Staff (where students are repeatedly engaging in behaviors such as using threats, teasing, gestures, putdowns, obscenities, and/or physical contact) G + L to P
Physical, verbal, emotional or sexual harassment to a student G + L to P

A. Verbal warning
B. Natural consequence to rebuild trust (Such as: written apology, pick up litter, go back and walk in hallway, removing graffiti, return of property, restitution/restoration required (will reimburse school district for cost of
repair)
C. Time out
D. Student-teacher conference
E. Student-administrator conference
F. Loss of recess
G. Restriction of privileges
H. Discipline referral sent home and signed
I. Phone call to parent/guardian
J. In-school Supervised Study
K. After School Detention
L. 1 to 2 days In-school Suspension (ISS) or Out of School Suspension (OSS)
M. 3 to 5 days In-school Suspension (ISS) or Out of School Suspension (OSS)
N. 10 day Out of School Suspension (OSS), student discipline hearing (possible recommendation for expulsion)
O. Reinstatement Meeting
P. Police notification mandated
Q. Restricted bus privileges
R. Notify Truancy Officer
