Health Examinations and Immunizations
Kindergarten and sixth-grade students, as well as all new enrollees from out of state, are required to have a health examination and immunizations.

All Illinois children in kindergarten and Grades 2 and 6 are required to have an oral health examination. Examinations must be performed by a licensed dentist, and he/she must sign the proof of school dental examination form. Each child is required to present proof of examination by a dentist prior to May 15 of the school year.

All children enrolling in kindergarten or in school for the first time are required to have an eye exam.

Lead screening is recommended for children under six; consult your doctor. All Head-Start children are required to have lead screenings completed.
These forms are also available at each school and at the District Office. Students not in compliance with the state-mandated health examination and immunization requirements will be excluded from school until such compliance.
Asthma Action Plan
Governor Rauner recently signed Public Act 099-0843 into effect. The Public Act requires school districts to have an Asthma Emergency Response Protocol in place, provide training to staff on asthma symptoms and treatment every two years, and ask parents of students diagnosed with asthma to provide the school with an Asthma Action Plan annually.
If your child needs to receive medication at school or will be self-carrying/self-administering asthma medication at school, please have the school medication form completed by your child’s physician and complete the parent section.
